The Slezak or Slezák surname comes from the Czech designation of the inhabitant of Silesia (Slezsko), according to onomastic research. Although variations amongst bearers of the perennial and distinguished Slezak surname may exist, the archetypal form of Slezak is esteemed. The Polish equivalent of that name is Ślęzak.
