The season premiere aired on September 16, 2006.

Season 12 went by with only a few minor cast changes to the show. All of the remaining cast members of season eleven (with the exception of Michael McDonald and Bobby Lee), were under contract to appear in Season Twelve. However, both decided to renew their contracts, so the only cast members who left the show between seasons eleven and twelve were Stephnie Weir (who left midway through Season Eleven), Frank Caliendo (who chose not to renew his contract), and Daniele Gaither (who was let go by FOX executives). Because of Frank Caliendo's departure, there is a significant lack of political sketches (as Caliendo was the only castmember who could impersonate George W. Bush) and an uptick in TV and movie parodies, celebrity-based sketches, and recurring character sketches to make up for the loss.

There was also a major format change in Season 12. At the beginning of the show, instead of airing a sketch before the opening credits, sketches from the upcoming episode are previewed. Also, so more sketches can fit into individual episodes, sketches are shorter, much like they were in early years. Often, sketches that have too much material to be all in one sketch are split into 3 parts, to prevent the sketch from dragging too much (such as a three-part filler piece about what shows celebrities have saved to their TiVo).

The featured performers from Season Eleven, Frank Caeti and Nicole Randall Johnson, were promoted to repertory status.

New sketches in Season 12 included a Korean Soap Opera spoof called Attidues and Feelings Both Desirable and Sometimes Secretive, starring Bobby Lee. Attidues and Feelings, spoken in fluent yet random Korean words and subtitled in comical Broken English (the dialogue and subtitles are not related), is a rather bizarre cliched soap opera about the twisted life of Dr. Poon Jum, his love's antics, and his enemy, Mr. President, a Korean crimelord. Each sketch left the viewer in a hilarious yet shocking cliffhanger. The sketches were usually shown about 5 episodes apart. Lee also played Johhny Gan, a Korean millionaire who hosts a rather odd talk show where him and his assistant, Pongo (who seems to be in love with him) interview celebrities. Also, Michael McDonald brought over a character from season 11, Larry Felmer, a celebrity-obsessed man who gets extremely angry when celebrities are mocked. Familiar faces also returned in Season 12, including Crista Flanagan's Wendy Walker (Three Minute Meals) and Luanne Lockheart, Nicole Parker's Disney Girl, Nicole Randall Johnson's Ka-Son, Keegan-Michael Key's Coach Hines, and Michael McDonald's Stuart.

For the first time in 7 years, animated sketches appeared on MADtv. Season 12 saw the return of claymation to the show with the recurring sketch "Celebrity Pets", in which pets of celebrities talk about their daily lives with their celebrity owners. There were also some non-sequitor claymation sketches such as "Red Snow" (a Happy Feet-meets-Saving Private Ryan movie trailer), "Cupid", Real Police Video: Immigration, and a Christmas short where George W. Bush blackmails Santa into killing his enemies. MADtv also developed a popular recurring animation sketch called "Weekly News with Toby", in which first grader Toby (voiced by Frank Caeti) talks about his view on current events.

Because of the major format change, shorter sketches, and return of claymation and animation, many have agreed that Season 12 has brought back the feel of the early seasons of MADtv, and some have also dubbed it a new era for the show, similar to how Saturday Night Live's twelfth season (1986-1987) was considered to be the show's new era after Lorne Michaels got rid of most of season 11's critically-panned cast.

The twelfth season features a brand-new title sequence showing action shots of the performers as they prepare for the show. The screen divides into several different live action shots. When the music starts, the announcer says "It's MADtv! Starring...". When the last cast member is announced, the screen turns to the whole cast in a live action shot as the music stops and the phrase "You are now watching MADtv!" is said. The sequence also features a new announcer.

Maury Povich (Michael McDonald) and Connie Chung (Bobby Lee) star in their own reality show. Ellen DeGeneres (Nicole Parker) is the new spokeswoman for adopting Iraqi children. BET premieres a music video in which Jordan Peele sings about his love for video games. In a Terry Gilliam-esque animated piece, Suri Cruise pleas for help from Tom Cruise's clutches on the cover of Vanity Fair magazine. Larry King makes long, lewd, and crude comments in his reviews for Lost. On 24 with Bobby Lee, Deal or No Deal host Howie Mandel teaches Bobby Lee how to be a better comedian and repeatedly cleans him. An ugly girl (Crista Flanagan) with magic powers brings good luck to everyone but herself. Arden Myrin interviews celebrities at the red carpet for the nip/tuck season premiere party. Former presidential nominee John Kerry (Michael McDonald) tries to clear the air about his recently controversial "botched joke". Nelly Furtado gets hounded by Eugene (Keegan-Michael Key) and tricked into making a bootleg for him. This week's installment of The Weekly News with Toby involves Rush Limbaugh's recent comments about Michael J. Fox. A single mother (Nicole Parker) wishes for the days before she had a child in a commercial for the RU486 morning after abortion pill. The new Truman Capote film Infamous features inconsequential footage, while other variations have Mark Wahlberg (Ike Barinholtz) and Morgan Freeman (Jordan Peele) as the main character. Coach Hines (Keegan-Michael Key) gets a surprise homecoming visit from a former high school football player (Jordan Peele), who digs up some dirt from the coach's past. During a PGA tournament taking place at a desert, sports announcers (Keegan-Michael Key, Michael McDonald), as well as pro golfer Ted Graham (Ike Barinholtz) and his caddy (Frank Caeti), suffer from heat exhaustion. A trailer for Dreamgirls mocks the film for being derivative of other films about musicians struggling with fame and popularity. The newest Grand Theft Auto video game has all the action take place in Kansas. Stuart Larkin (Michael McDonald) wins a free shopping spree at a candy store. In 1314, a Scottish warlord (Jordan Peele) can't convince his men that he was raped by the British. Despite skepticism from one of their friends (Frank Caeti), the Superstitious Knights (Keegan-Michael Key, Jordan Peele) do their victory dances in an effort to help the Chicago Bears win the Super Bowl. A news station reports on supposed terrorist attacks and reports of AIDS-carrying puppies. Mofaz the Depressed Persian Tow Truck Man (Michael McDonald) and Condoleezza Rice (Nicole Randall Johnson) grouse about how security-obsessed America has become. Ike Barinholtz, Keegan-Michael Key, and Michael McDonald discuss who will win this year's Super Bowl. American Idol host Paula Abdul (Nicole Parker) reports live via satellite to clear the air about recent events, but she constantly confuses Keegan-Michael Key with the names of other actors. 24 star Mary Lynn Rajskub accidentally wakes up next to Bobby Lee, confusing him with Heroes star Masi Oka, on this week's 24 with Bobby Lee. During a speech by Hillary Clinton (Nicole Parker), everyone clamors over the presence of presidential nominee Barack Obama (Keegan-Michael Key). A man (Michael McDonald) tries to propose to his girlfriend (Nicole Parker), but both have to endure emu droppings. The Blind Kung-Fu Master (Bobby Lee) is sent to space to stop an alien princess (Crista Flanagan) from dominating the universe. Bobby Lee asked Arden Myrin to be his wing lady, so he can get attention from girls at the bar. Celebrity Pets focuses on Dick Cheney's perturbed dog, who warns viewers about the VP's plans on world domination. The Seattle Grace Hospital doctors from Grey's Anatomy call upon Dr. Gregory House (Michael McDonald) to cure a pregnant woman who's foaming at the mouth. Rapper Flavor Flav (Jordan Peele) warps great moments in African American history, much to the chagrin of Sidney Poitier (Keegan-Michael Key). In a parody of Freedom Writers, a white teacher (Nicole Parker) is the only person to help juvenile delinquents on the path to redemption. On another installment of Attidues and Feelings Both Desirable and Sometimes Secretive, Dr. Poon Ji-Sum (Bobby Lee) learns that his former lover has a twin, and he is forced to confront a Canadian crimelord (Edge). Darrell (Nicole Randall Johnson) bothers another white girl (Crista Flanagan) at a movie theater into giving him her phone number. A newly married couple (Michael McDonald, Nicole Parker) share bizarre stories with other married couples about how they met, but get constantly interrupted and mocked. The Jazzed for Crafts salesladies (Crista Flanagan, Arden Myrin) sell DVDs which catches the attention of a possible suitor (Frank Caeti).
