Franciscan missionaries arrived in June 24, 1578. These included Fray Juan Clemente who studied the medicinal value of plants. They created a dispensary in Intramuros. In the same year, Fray Clemente built a bamboo hospital with two wards. In 1590, this was expanded to 3 wards with the arrival of Fray Juan Fernandez de Leon who became the first chaplain.

The Confraternity of Mercy (Hermanidad de la Misericordia) was founded in April 16, 1594 to solicit funds and eventually took over the hospital in 1596. After numerous changes in management, the hospital in 1952 eventually relocated to Pasay City, on Roxas Boulevard an extension of Coastal Road then known as Dewey Boulevard.
