All requests must be created using the inputbox below, and must utilize the preloaded template without exception!

Please read all the instructions before attempting to file a request. Guide to filing a Request for Mediation. Sample Request for Mediation with instructions. (To actually file a request, use the Mediation request box below.) Common reasons Requests for Mediation are rejected. If you have questions about how to file a request, please feel free to ask the Committee chair.

You must: Add the text {{RFMF|Case Page Name|~~~~~}} to the top of the talk page of all involved articles. "Case Page Name" should be the same name you put in the box below. Add the text {{RFM-Request|Case Page Name}} to the talk page of all involved parties. "Case Page Name" should be the same name you put in the box below. Watchlist the request page; if additional information is required, you will be asked there, and expected to respond within the 7 day period. You must not: Attempt to hold a debate on this page. Discussions take place after acceptance, not before: There is no need for, desire to read, or acceptance of lengthy debates on the merit of mediation. If you wish to debate on whether to mediate, do it on the article's talk page, not here. Move or remove any content on the mediation pages under any circumstances. Content removal is restricted to members of the Mediation Committee.

All parties must sign the request within 7 days or the case will be rejected.
