Wikipedia:Requests for mediation/Sample

This page is intended as an example of what a request should look like. The template for requests, Wikipedia:Requests for mediation/Top/Sample, should be used for all requests (it is pre-loaded when using the inputbox on RfM). This page is a guide, with additional comments in blue to offer advice on filling out the template.
Important: If you have questions or are confused at all, please feel free to ask the Committee chair.

Case name

Title for case: insert the name of the article between the == == above, and affix 2, 3, etc. if there are previous mediations by the same name.

Involved parties

Add a bullet and the {{user|Username}} template for each party to the mediation. (Where "Username" is the user's actual username.) Parties are defined as individuals who should be included in the mediation discussions; it is not necessary to list every user who has edited the article, but it is essential to include all editors involved in the matter that is being mediated.

Other steps in dispute resolution that have been attempted:

If an RfC was filed, provide a link. If the issue was raised on the Administrators Noticeboard or other similar page, provide a stable link (one that links to a revision in the history that contains the full text of the discussion, since the AN is archived regularly). If informal dispute resolution, such as Wikipedia:Third Opinion, were attempted, provide links. Do not provide links to discussions on the article's talk page(s), or to discussions on user talk pages.

Please briefly statet the issues to be mediated as bullet points. Only state issues to be addressed (Should Foo contain a link to www.foo.com?); do not comment on user conduct (User:Example has been edit warring on Foo, inserting links to www.foo.com). This is not the place to explain the entire history of the dispute; each party will have the opportunity to make a full statement once the mediation is accepted. If you do not state the issues to be addresssed, the mediation will not be accepted; discussion of user conduct will be removed by a member of the Mediation Committee. Excessive or spurrious commentary will also be removed. (Note: Only committee members may remove text from this page; editing or otherwise altering any content on this page is vandalism.)

This section is for the other party to add any issues that are not included above. User bullet points to add additional issues. This is not a response to the issues set out above, nor is it an opportunity to make a statment of the party's opinion, describe another user's actions, or state the history of the dispute. Only issues are to be added; as above, commentary on persons, rather than issues, will be removed by a member of the Mediation Committee. Excessive or spurrious commentary will also be removed. (Once again, only committee members may remove text from the RfM page.)

Each party should provide the issues to be mediated in a separate section; if there are three parties, then there should be three "issue" sections. Editing of another party's section is not allowed. If any party does not agree to mediate, it is not necessary to fill out these sections; simply indicate "Do not agree" and sign in the section below. If any party refuses to mediate, then the mediation will be automatically rejected.

Parties' agreement to mediate

All parties should sign below, indicating that they agree to mediate the issue. If any party fails to sign, or if a party indicates they do not agree, then the mediation will be rejected.

Only signatures should go here, along with either "Agree" or "Do not agree." Any additional comments will be removed by a member of the Mediation Committee.

This section should be inserted, but left blank; when the committee decides whether to accept or reject the mediation, the decision will be posted here. If the request is rejected, reasons for rejection will be included here. If it is accepted, it will be moved to a subpage of Requests for Mediation. If the decision is extended pending further information, the information required will be listed here.
