Pilot (alternate titles: McNamara/Troy[1] or Silvio Perez) is the first episode of the American television drama Nip/Tuck. It aired on July 22, 2003.

"Tell me what you don't like about yourself..." says Dr. Sean McNamara to his current patient, a Hispanic man named Silvio Perez. Silvio, however, doesn't speak English, and Sean doesn't speak Spanish. He feels uncomfortable having Silvio's brother, Alejandro, speak for his brother in light of the extensive surgeries he requests. So, he interrupts his partner, Dr. Christian Troy, in surgery to have him find out what's going on. Christian, who speaks Spanish, finds out that Silvio is "running" after he slept with the boss's girl. He suspects that Silvio and Alejandro are in involved in a drug cartel, and initially refuses to do the surgery, though reconsiders when the pair offer him $300,000. Sean, who cannot speak Spanish, is completely unaware of this transaction, and Christian tells him that Silvio just wants "a fresh start".

Later, Christian goes out and picks up a beautiful woman at a tennis club (we find out later that her name is Kimber), and returns to his apartment with her, where he performs anal sex and snorts cocaine off her back at the same time. Meanwhile, Sean and his wife Julia are having very conventional--and very boring--sex. After Sean finishes, Julia is clearly annoyed, and we can see the tension in their marriage immediately. After Christian finishes, he leads Kimber to believe that she is an "8" and not a "10". He marks her with a lipstick, and indicates that her problem areas should fuel her.

The McNamaras two children, Matt and Annie, play an important role in the parents' relationship. Annie is a quiet 8-year old, where Matt is a brash 16-year-old. He is very close friends with Christian, and is angry with his father because he will not give him the circumcision he feels he needs because he is concerned what his girlfriend, Vanessa, will think when they have sex. Kimber comes to the McNamara/Troy office after getting plastic surgery at the suggestion of Christian, but he is nowhere to be found. Kimber thought Christian loved her, but Liz (the staff anesthesiologist) informs her that she was just one of a large number of girls to have this happen.

After performing the Perez facelift and learning about the $300,000 transaction, Sean confronts Christian about it, even going so far as to state that he is going to leave the practice. When news of this leaks back to Julia, she goes ballistic, and has an incredibly intense confrontation with Sean in the kitchen, throwing a drinking glass at his head, which barely misses him as it smashes against the wall. Sean moves out and into his new office, a shanty hole-in-the-wall type of place.

Christian, meanwhile, continues business as usual. He is summoned to release Silvio from the hospital following his surgery, but when he goes to find him, finds he is not in his bed. Alejandro takes him to the children's wing of the hospital, where they find him feeding his ice cream to a 6-year-old girl. Because of how he acts towards the girl, Christian asks how old the "boss's girl" was, only to find out that she was 6 years old.

Later, Christian is called to a "Botox party" for ten women in a very fancy upper-class penthouse apartment. After Botoxing all ten, a huge bodyguard enters, followed by a very angry man--Escobar Gallardo, Silvio Perez's boss, the man whose daughter was raped. In the ensuing torture executed by Escobar to find Sean and Silvio, Christian is injected with many shots of Botox in his cheeks, motor control sites, and his penis. Sean finally calls Christian's cell phone after the last shot is injected, where Escobar learns that Silvio is with Sean. They go to meet up with him.

Sean is with Silvio because Silvio offered him $20,000 cash to give him liposuction to go with the facelift. During the operation, Alejandro interrupts by pointing a gun, demanding that his brother be woken up. Even though the pair (Sean and Liz) insist he could die, Alejandro gets them to wake Silvio, where he tells him that his girlfriend is pregnant, but that Silvio will never get to see his daughter. He then kills Silvio by turning the nitrous oxide levels on the anesthesia machine to fatal levels.

Escobar and Christian, enter this scene and Escobar leaves with Alejandro. Sean and Christian, now left with the problem of getting rid of the body of the now deceased Silvio Perez, go to the supermarket. They buy hams, tie them to the arms and legs of Silvio, and dump him in the Everglades to be eaten by alligators.
