State Route 32 is a major thoroughfare across the southern portion of Ohio. Its western terminus is at U.S. Route 50 along with State Route 125. State Route 561 begins here and continues west. About a mile later, State Route 32 splits from State Route 125 and heads east through the eastern suburbs of Cincinnati. Just east of Interstate 275 it becomes a limited-access highway. Its eastern terminus is in Belpre at the eastern terminus of the concurrent section of U.S. Route 50/State Route 7, where State Route 7 continues north and U.S. Route 50 briefly heads south before crossing the Ohio River to enter Parkersburg, West Virginia.

OH 32 was previously labeled OH 74, but was renumbered in 1962 to avoid confusion with Interstate 74. (In Ohio, U.S. and Interstate highways are also classified as state routes, so the number "74" could not be used for both.) This can still be seen today, as "Old State Route 74" can be found running parallel to some of the newer divided highway segments of OH 32, especially in the Batavia/Eastgate area.
