State Route 159 is a northeast-southwest state highway (signed north-south) in the south-central portion of the U.S. state of Ohio. Its southern terminus is in Chillicothe at the U.S. Route 50 and State Route 104 duplex; its northern terminus is at U.S. Route 22 approximately 4 miles southeast Lancaster.

- 1924 – Original route established[2]; originally routed along its current alignment from 6½ miles north of Chillicothe (at current State Route 180) to the former alignment of U.S. Route 22 1 mile east of Amanda[3].
- 1926 – Extended south to 1 mile north of Oakland along the alignment of former State Route 27[3].
- 1970 – Extended north to its current terminus along the former alignment of U.S. Route 22[3].
- 1972 – Extended south to its current terminus along the former alignment of U.S. Route 23[3].
