i think you are being selfish in taking a position of opposition to global warming...many people on the planet would greatly benefit such as in canada or russia...i think you need to recognize that many scientists are taking a position that global warming is occuring...yet it helps many people out...it can actually be a good thing...we just need to figure out what to do with the polar bears...Benjiwolf 10:55, 29 January 2007 (UTC)

I don't know if you caught my error, but I said in one of my edit summaries on the Schiavo page that I reverted your edit, but in fact it was a sweeping edit by another user, and I made a typo:

Apparently, our fingers slip up once in a while, and since I can't change my edit summary, I figured to at least apologise to you, the person wrongly named -and no offense meant either to the other editor, but I was ticked when I saw her edit, and ...uh...made that typo, lol. While I'm happy to share my knowledge as an researcher, I would preface by saying that any medical decisions should be made in consultation with your doctors who are likely to know a lot more about your specific situation.

Here in Australia, bisphosphonates are being increasingly used to treat OI to great benefit. Bisphosphonates decrease bone resorption leading to an increase in bone mass. However, they do not improve bone quality, which is why OI bones are brittle and break. Thus the decrease in fracture rate that is achieved with bisphosphonates is because the bone is larger, not "better" (does that make sense?).

Bisphosphonates have been getting a lot of bad press lately because of osteonecrosis of the jaw, which is a rare complication pretty much only seen in cancer patients on high dose bisphosphonates who have dental extractions. While I think OI children are a low risk of this, I'd talk to your doctor if you are on bisphosphonates and are getting a tooth removed. I haven't heard any reports of ONJ in children with OI, but that doesn't mean it can't happen.

A colleague of mine who was involved with the Pamidronate work in Canada looked at bone turnover in OI children on long term Pam and found a large decrease in bone turnover. What he found was that while resorption was suppressed (which is good), over many years bone formation also went down (which may not be so good). Clinicians are now thinking about introducing "bisphosphonate holidays" into dosing schedules, to allow the bone formation side of things a chance to recover. Of course, I don't know of any formal programs that do this yet.

I wouldn't be too concerned about Pamidronate treatment. It is becoming increasingly common in many countries and at our hospital bisphosphonates are routinely given off-label to children undergoing orthopaedic procedures. In terms of side-effects, it is common to have flu like symptoms and dizziness after the first infusion. But this is due to gamma-delta T-cells (an immune response) and only happens the first time - afterwards, there should be no problems. The reduction in fracture rates with Pam can be quite dramatic.
