New York State Route 64 is a north-south running state route located in the counties of Monroe and Ontario in Western New York. The northern terminus of NY 64 is at New York State Route 252 and New York State Route 96 in Pittsford. The southern terminus is located at New York State Route 21 in Bristol Springs.

NY 64 begins at an intersection with NY 21 on a ridge overlooking Canandaigua Lake in Bristol Springs, a hamlet of Bristol. Heading to the northwest, NY 64 runs in the base of the Bristol Valley, a valley created by a pair of imposing mountain ranges to the east and west. At Bristol Center, roughly 7.5 miles from Bristol Springs, the western mountains give way to flat farmland while the eastern range continues on, although to a lesser extent and a lower elevation than before. The elevation of NY 64 remains virtually unchanged, however, as it progresses northward. 1.5 miles from Bristol Center, NY 64 meets U.S. Route 20A.

US 20A and NY 64 join together northward for 3.75 miles, allowing US 20A to terminate at its parent, U.S. Route 20, running concurrent with New York State Route 5 here, at an intersection in East Bloomfield. At this point, NY 64 turns west onto Routes 5 and 20.

Routes 5, 20 and 64 then meet the southern terminus of New York State Route 444, the former eastern terminus of New York State Route 20C, south of the former village of Holcomb, now a hamlet of East Bloomfield. The concurrency continues westward for roughly two miles before NY 64 breaks from NY 5 and US 20 and turns northward toward Rochester once again.

A mere 50 yards north of NY 5 and US 20, NY 64 meets the west end of West Main Street at a mediocre rural intersection. Although no longer shown with the exception of an oversized, and mostly empty, sign assembly, this intersection was the western terminus of NY 20C until 1990, when NY 20C was taken off of West Main.

North of West Main, NY 64 curves noticeably to the northwest as it exits East Bloomfield and enters its directional counterpart, West Bloomfield. In the northeast corner of the town, NY 64 passes through the hamlet of Ionia. Northwest of the community, NY 64 breaks to the north, taking a due north alignment as it passes into Monroe County.

Upon crossing the county line, NY 64 becomes a named road for the first time, gaining the name Mendon-Ionia Road. The road stays on a relatively straight path northward as it heads toward Mendon, where NY 64 meets New York State Route 251 in the village center. On the opposite side of NY 251, NY 64 becomes Pittsford-Mendon Road.

Outside of the village, NY 64 maintains a consistent northerly alignment as it runs through the town of Mendon and passes over the New York State Thruway (Interstate 90). The route enters Pittsford a short distance north of the Thruway, at which time NY 64 becomes Mendon Road.

Directly south of the village of Pittsford, NY 64 enters an intersection with South Main Street, Stone Road and Mendon Center Road at a sharp angle, creating difficult turning angles to and from Mendon Road. NY 64 turns north onto South Main Street, where it remains to its northern terminus at NY 96 and NY 252 at the junction of South Main Street and Jefferson Road.

Prior to 1983, NY 64 was part of three concurrencies. The third concurrency was located between NY 64's northern terminus in Pittsford and Mendon Center Road as NY 64 shared South Main Street with New York State Route 253. This concurrency, which was an unnecessary one considering that NY 64 ended in Pittsford while NY 253 continued north on NY 96, ceased to exist when NY 253 was truncated to New York State Route 65 in 1983.
