Neifirst (ネイファースト Neifāsuto?) is a major villain and plot element in the Phantasy Star II computer role-playing game.

In the fictional time period AW 1282, Neifirst was the result of an accident in the Biosystems Lab on the planet Motavia; she was the first Numan (a hybrid of biomonster and human). Shortly after her accidental "birth" she was deemed a failure and her creators tried to kill her. She escaped the Bio Systems Lab with stolen Dna and infiltrated the Climatrol Centre. Using the vast energies within the system she caused a massive biomonster outbreak to begin, wreaking revenge upon the inhabitants of Motavia. On August 30, AW 1283, Nei, the embodiment of compassion within Neifirst, dichotomized herself from her host and escaped, eventually being adopted as a sister by Rolf Landale and taken to the capital city Paseo.

Accompanied by Nei, Rolf Landale was dispatched to discover the cause of the biomonster outbreak which lead to him finding Neifirst at the Climatrol Centre, reuniting the two "sisters". Nei attempts to defeat Neifirst on her own, but more than likely (this is dependent on how prepared the player is) she is struck down, prompting Rolf and any other current party members to continue the attack. Whichever way this event unfolds it brings upon the death of both Neifirst and Nei.

Neifirst is found in the middle of Climatrol's eighth floor. She has 1,100 hit points, an attack power of 178 and a defense power of 10. Her attacks consist of a giant claw that attacks all party members and an energy composed throwing blade that deals a large amount of damage to a single member. Defeating her earns the characters 11,111 experience and 1,111 Meseta.

In the remake for the Playstation 2, Neifirst plays the same role as she does in the original. In battle, she now has the ability to heal herself with the techniques Gi-Resta and Na-Resta. She is easier to beat with Nei's new abilities, however if Nei is killed Rolf battles her alone without the rest of the party, unlike the original. She has 1,500 hit points and the party receives 1,111 experience points and 55 Meseta.

In Neifirst's original design she was portrayed wearing a heavy suit of armour, purposely to conflict her design with that of Nei's minimal leotard outfit. This design never made it into the original version of Phantasy Star II and Neifirst ended up having a similar design to that of Nei, However the re-design featured in "Generation 2" has incorporated some of the armour elements.
