The Janitor installs a toilet on the roof of the hospital, claiming that he can look at his life better than if he used a "germ-infested" stall in the building. These life-changing thoughts give the toilet the name of the "epiphany toilet."
The episode features, for the second time, Dr. Kevin Casey (played by Michael J. Fox), who is an amazing doctor in both the fields of medicine and surgery, but he has a severe case of OCD.
Turk asks J.D. to be the best man at his wedding, but J.D. hears a message from Turk's brother that reveals that Turk had asked his brother to be best man first. Turk later explains that he only asked his brother because he knew that his brother wouldn't be able to make it.
Elliot is afraid of intubating patients and seeks help from Dr. Kevin Casey. The doctor does help her, but in the form of an epiphany while she is on the Epiphany toilet.
Dr. Kelso shuts down an entire wing of the hospital in order to save a large sum of money, but many patients are arriving and there are not enough rooms to accommodate them. When Bob Kelso asks Dr. Cox and Carla to find a room for his gardener, Dr. Cox turns Dr. Kelso's office into a room for the gardener. Kelso eventually re-opens the shut-down wing.
