Elliot demands single-sex change rooms from Dr. Kelso as it's becoming increasingly embarrassing changing in the traditional mixed-sex rooms.

Turk wants to pop the question to Carla, but this does not work out in the way he had originally planned: Carla's aunt Marie dies and the ring almost gets lost in the a sink, then it is eaten by a young patient. When Carla leaves for her aunt's funeral, he asks her without a ring. Both of them are surprised that her answer is "I need to think".

Meanwhile, Dr. Cox and J.D. have to deal with two patients; Mrs. Larkin is a pregnant woman with heart problems whose sensible husband is anything but a help. The other patient is Elaine, a young woman who waits for a heart transplant. She becomes friends with J.D. and they talk a lot.

The episode ends with Mrs. Larkin's baby being born, mother and son being all right. But only moments later, the doctors storm into Elaine's room, her heart monitor beeping wildly. J.D. daydreams a scene Elaine had imagined for her death: she stands next to her bed, dressed in a lovely scarlet gown and bathed in the glow of theatrical spotlight. She begins to sing Colin Hay's "Waiting for My Real Life to Begin", with several staff members joining in.

When J.D. wakes up from the daydream, Elaine has died. Tears well up in his eyes as he stares at her. When asked by Dr. Cox if he will be alright, he quietly responds "...yeah."

  • When Turk is searching the kid's mouth for the ring that the kid had just swallowed, the ring can still be seen in his mouth after Turk yells "No, you did not just swallow that thing!"
  • Janitor refers to J.D. as "Scooter". The nickname he gave him in the episode "My Nickname"
