"My Old Lady" is the fourth episode of the American sitcom Scrubs. It originally aired as Episode 4 of Season 1 on October 16, 2001.

The three interns have to face death for the first time: J.D.'s patient, Mrs. Tanner, is a headstrong but kindly old woman, Turk's is a young man called David who just needs some company, and Elliot's is Mrs. Guerrero, who speaks no English. Elliot and Carla, despite a bad beginning, start to become more friendly, until Elliot's anxiety gets the better of her.
In the beginning of this episode, J.D. says that one out of three patients dies in hospital. This suggests that one of the three interns' patients will die. In fact, all three patients are dead at the end of the episode, and their doctors have to tell the bad news to their patients' loved ones. As the late Mrs. Tanner had suggested, J.D. ends up lying in the grass and doing nothing, just thinking about all the things he wants to do in life.

- The short clip of J.D. with Death is a reference to a Connect Four commercial from the 90's.
- Scrubs writer Matt Tarses won the 2002 Humanitas Prize in the 30 Minute Category for this episode.
- When J.D. and Death are playing Connect Four, J.D. has eight tokens, whereas Death has only six. This could be an indication of JD trying to cheat death.
- When Turk's patient, David, asks if anyone has ever heard of "the Catch," Turk comes in and says "Joe Montana to Dwight Clark, NFC Championship game, no time left on the clock." There were actually :51 seconds left on the clock when the catch was made.
- When Turk says he's just going to go ahead and take David's fruit cup, he actually takes David's Jell-O. The fruit cup can be seen on the other side of his tray.
- According to Zach Braff, he was mad at Sarah Chalke during the filming of the episode's final scene. When Carla, Turk and Elliot joins J.D as he relaxes in the park, Zach interacts with the other two actors and doesn't look at Sarah.
