The end of their second year has the residents somewhat bored with the monotony of their jobs, until Spence, an old college buddy of J.D. and Turk's turns up and accidentally reveals to Dr. Cox that Jordan's son is also his; further antics have J.D. and Turk dismissed from their on-call shifts when they arrive after drinking with Spence. Meanwhile, the Janitor starts another feud – this time over paychecks – with J.D.
As Dr. Kelso attempts to break Elliot's spirit, Dr. Cox tries to cope with the shock that he is Jack's father, but Jordan states that she can't lose him again after just getting him back, and keeps following him everywhere. Cox eventually confesses to J.D. that he is not even angry at Jordan for lying to him; he is scared that he will be a bad father. J.D. reassures him, pointing out how everyone at the hospital looks up to him, and the way he disciplined them for drinking on a work night. This prompts Cox to intervene when he sees Kelso publicly humiliating Elliot; he walks over to them and punches the Chief of Medicine in the face, leaving everybody stunned. It is revealed in a later episode that Cox broke Kelso's nose.

- When Elliot is talking to Ted about Dr. Kelso, Ted has a keyboard for a computer on his desk. However, there is no CPU or monitor on the desk
- When J.D. imagines being the "Chocolate King" and pulls off Turks right hand, Donald Faison's real fingers can still be seen in his sleeve.
