As J.D.'s hate for Keith increases, so does his like for another intern, Jason, whom J.D. calls Cabbage. Although Cabbage is the worst of the interns, J.D attempts to keep him and rid himself of Keith. Turk and Elliot realize that their friendship was not as strong as they thought it was when a dead patient leaves a letter for his twin sons, and they lose it. Everyone's favorite patient, Mrs. Wilk, is about to leave the hospital at this time. Janitor grows fond of a crow, Sanchez, which Dr. Kelso wants out of the hospital for fear of the bird spreading infection. At the end, Elliot and Turk get the note from Sanchez, and Turk and Elliot become closer as friends. Sanchez is released by Janitor to fly into the sky. Although it is hard for J.D., he has to kick Cabbage out of the hospital. At the very last scene, Cabbage is walking out and throws something with infection all over it, in the garbage. He does not bother to wash his hands and says goodbye to Mrs. Wilk because she had always been so nice to him. When he gives her a hand shake, there is a green glow along her body which represents infection, resulting in her death in the next episode.

In this episode, J.D. and Turk remember something they had mentally blocked for twelve years and agree to not talk for "a few weeks." This lasts throughout this episode and through the next.
