J.D. must look after a social worker, Alex Hanson, who slipped and fell in the hospital and winds up stuck in an MRI machine. Afraid she might sue the hospital, Dr. Kelso wants him to stand by her and be friendly. They flirt, but J.D. is reluctant to ask her out without knowing what she looks like.
Elliot tries to gain Dr. Cox's favor the way J.D. has during a 24-hour marathon to keep his ICU patients alive. Meanwhile, Turk snaps at Carla when she steals fries from his plate. They realize they are experiencing some relationship problems, and Carla expects Turk to end their relationship, but in the end he tells her that he loves her.

- Turk and Carla first express their love for each other in this episode. This will be a problem for their lovelife in the episode My Balancing Act, but lead to a long-term relationship in the seasons to come.
