[edit] Episode overview

Spoiler warning: Plot and/or ending details follow.

All of the doctors at Sacred Heart are being questioned about the mysterious death of a patient. JD, the patient's doctor, and Turk went to get a former patient of JD's to say "Thank you". JD asks Elliott to cover for him, but Elliott makes out with a man who was married, so she becomes busy running away from the man's wife. While all of this is going on, Carla gets the Janitor, Ted and the entire nursing staff to buy lottery tickets.

After JD and Turk go to the patient's house and demand a thank you, the patient doesn't give it to them, because he say that he's a garbage man and garbage men don't get thanked very often. At the hospital, the interns need Turk, because surgeons also know how to deal with the patient’s case, and Turk is the on-call surgeon.

While JD sits near the street to thank the garbage man, Turk leaves to go to the hospital, only to get stuck in traffic. Elliott is taped to a wall by the man's wife, and the entire nursing staff is watching the lottery. Meanwhile, the patient was left to die in the hands of the interns.

Then, back at the conference, Doug runs in to show that he didn't die because the doctors weren't there; he died because of a pulmonary embolism that was unnoticed by the radiologist. Now the only one to blame was the head of the radiology department, not JD, Elliott, Turk or the nursing staff. Dr. Cox later confronts JD, Turk, Elliott and Carla, who are celebrating that it wasn't their fault. After commenting on how they should make a toast to the patient's widow and fatherless children, he tells them not to celebrate because it could've been them, they just lucked out this time.

Spoilers end here.

[edit] Trivia

  • J.D. and Turk’s encounter with a group of violent ostriches is a spoof of a scene in the film Dude, Where's My Car?
  • Dr.Cox's "10 second" rant about the odds of winning the lottery is actually 15 seconds
  • It is rumoured that Jason Bateman's appearance was supposed to be bigger, but was reduced when he had to have throat surgery. He appears in two scenes in the whole episode, unusual for a guest star of his caliber.
  • This was the first episode to feature "Floating Head Doctor".
  • The name is reference to the ostriches as they are large birds.
