Mortal Dilemma / Get Out of My Head! is the 3rd episode of The Grim Adventures of Billy and Mandy. It first aired September 7, 2001 on Cartoon Network.

Grim is alone for a whole day, but he cannot reap while Billy and Mandy are at school. He ignores their demands, and attempts to reap a few souls. However, he is unable to. Thus, he decides to do some good deeds for a change, at which he fails miserably, mainly because nobody wants to be "helped" by the incarnation of Death.

Billy and Grim are watching a B-production horror film, featuring a monster which jumps into people's head and posseses them. Grim shows Billy how to jump into people's heads and use them as puppets. When Billy tries his new ability on Mandy, he proceeds to do silly stuff with her body throughout the week. To the bewilderment of others, Mandy chuckles and smiles as she eats worms, flirts with Irwin, hits herself with a hammer and eats her boogers. But finally, Billy is accidentally knocked out of Mandy's head, and has to confront her build up rage. At night, Mandy takes control of Billy's cat, Milkshakes, and exacts her revenge on Billy.
Get Out of My Head! marks the first appearance of Irwin: he appeares only briefly, taking a dog for a walk, when Mandy(being controlled by Billy) stops by to tell him that "he is a cutie." This episode might be a reference to the 1999 film Being John Malkovich

- In "Get out of My Head!", Ms. Butterbean is not the teacher of Billy and Mandy's class.
- Cow from Cow and Chicken can be seen on the milk carton in Mortal Dilemma.
- The Saying you can pick your friends and you can pick your nose, but you can't pick your friend's nose is later used in the episode Billy and Mandy VS. The Martians.
- At the end of the episode a magazine that Billy reads says Sassy Cat on it.
