The image linked here is claimed to be used under fair use as:

it is of low resolution (copies made from it will be of very inferior quality)
the photo is only being used for informational purposes.
Its inclusion in the article adds significantly to the article because the look and design of the card can be viewed
it would be difficult to take our own photo as we do not have access to "example" Medicare cards. Using real Medicare cards would be a breach of privacy.
