From the north, MD 351 begins at an interchange with MD 180 in Frederick near U.S. 340.
The route terminates at MD 28 in Point Of Rocks.

Maryland Route 351 served the areas of rural Frederick county lying southeast of the city of Frederick. Called Ballenger Creek Pike, it is named for Ballenger Creek, a small body of water which runs through the area. In the early seventies, this road was MD 78. Then it became part of MD 180. The MD 351 designation was applied to the route 1988. The route has always terminated in the same spot just north of Church Hill, concurrent with the terminus of MD 180.

At one point Ballenger Creek Pike carred the MD 180 designation from Frederick to Point Of Rocks. As a result the full route was semi-circular. This was changed when Ballenger Creek Pike was given the new route number of 351.
