From the west, MD 180 begins at an interchange with MD 351 in Frederick
The route terminates at U.S. Route 15 in an unincorporated portion of Washington County.

Maryland Route 180 carried the designation U.S. 340 prior to the construction of the freeway that now carries that route.

At one point Ballenger Creek Pike carred the MD 180 designation from Frederick to Point Of Rocks. As a result the full route was semi-circular. This was changed when Ballenger Creek Pike was given the new route number of 351.
