If you're dealing with someone who is a polymath like that, it can be difficult to use an infobox (see Martin Lewis for a case where the subject has so many disciplines no one has bothered to attempt an infobox) . The best suggestion I could give you is for you to visit Wikipedia:WikiProject Biography and put the question to the editors involved with that project. They might be able to provide some guidance. You could always just use a generic Biography Infobox (there's a link to the template from the Wikiproject page) rather than try and nail down a particular style. If your subject is considered preeminent in one of the fields you mentioned, then you could probably pick a single style of infobox. For example, Hedy Lamarr was a noted inventor as well as an actress, but because she is is more widely regarded as an actress, an Arts and Entertainment-related infobox is more appropriate than one used for inventors (although it appears only a generic infobox was actually used for her).
