Report a Vandal

I.
Warn User

Before adding a vandal here be sure the user has been warned.

On the user's talk page, warn that his/her behavior is not appropriate and is considered vandalism. Use the appropriate warning template, adding 'subst:' before each. For example, instead of using {{test}}, use {{subst:test}}. This reduces server load and prevents accidental blanking of the template. Wait to see if the vandalism continues. Only add users to the list who continue to vandalize after being warned.
However, users known to be vandals (those who vandalize the same article with similar material, for instance) may be reported immediately.
II.
Determine Severity

Determine the severity of the vandalism:

Severe: Hateful; critical subtle edits (such as changing dates); stating lies; redirects; etc.
Moderate: High-profile (such as article blanking); spam linking; disruptive; etc.
Low: "Stupid" edits (such as adding the word "penis" to every sentence); non-critical subtle edits; etc.
III.
Add Vandal

Using the edit button next to the appropriate section, add the vandal at the top of the list.

Enter the following text:
{{vandal|IP or username of vandal}} ~~~~
If the vandal has a username with spaces in it, instead use:
{{vandal|Username with spaces|Username_with_underscores}} ~~~~
If the vandal is operating under an anonymous IP address, report under IP Addresses. If the vandal is a logged-in user, report under Registered users. Include a brief description of the nature of the vandalism. Use the vandal account name or IP address as your edit summary.
IV.
Monitor Vandal

Determine that the vandal has been warned on his or her talk page. If vandalism persists after five minutes:

Determine if the vandal has been blocked by visiting Special:Ipblocklist.
If the vandal was blocked, but vandalism persists, file a new report under Possible sockpuppets.
If the vandal was not blocked, visit WP:AIV and post the report there.
