Phoebe's lifelong fear of the Woogyman in the basement is an ongoing source of amusement to Prue and Piper, until an earthquake unleashes a long-dormant shadow demon which turns Phoebe towards her dark side and against her own family. Much to Prue's dismay, Phoebe's erratic behavior and the bizarre deterioration of the house come to a head during a dinner party for a very important client of the auction house, who has a keen interest in the Halliwell mansion because she believes that it is located over a spiritual Nexus, a point of incredible energy that can be swayed either towards undeniable good or astonishing evil.

- The Woogyman: Originally thought of by the sisters as a story Grams told to help Phoebe sleep better when she was five years old, the Woogyman is in fact a shadow that was released during an earthquake. It resides, after such an earthquake, in the Halliwell's basement, a place known to be a powerful nexus between the five spiritual elements. The Nexus is a place that can be persuaded either good or evil, and the sister's ancestors built the manor on its location to claim it for good. It received its name because Phoebe thought it was a Boogie Man but, as a child, couldn't say the name properly. Hence, it was known as The Woogyman.

- The episode title is a play on words to the phrase Is there a doctor in the house?
- This is the first time we see Grams
- This is the first time the Nexus is referenced. It is later referenced when Cole tries to take in the shadows, as well as Zankou, but all fail.
- The Nexus was destroyed at the end of season 7 in the season finale, Something Wicca This Way Goes.
- Actresses Shannen Doherty and Jennifer Rhodes worked together previously in the movie Heathers.
- This is the first time the Charmed Ones add their own entry into their Book of Shadows.
- When Andy is about to leave the manor, Piper freezes him from outside, but it's been said previously that Piper must be in the same room for her to freeze someone as her powers were not that far advanced yet.
- In most Wiccan traditions, the elements are earth, air, fire, water, and sometimes spirit. Piper and Prue used water, wood, fire, earth and metal, which are the five Taoist elements and not the Wiccan ones.
- This is the first time Piper has ever frozen one of her sisters, namely Phoebe when she was influenced by the Shadow.
- For this episode only, Phoebe gains the power of conjuration, the ability to think of an object and make it appear in your possession. Notably, when Cole becomes The Source, he possesses the same power with the same black smoke effect. Students in Magic School (in Season 6) also possess this power.
- In later episodes the spell used to banish the Woogy is a shortened version of the original spell, which is: I am light, I am one too strong to fight, so go away and leave my sight and take with you this endless night
