[edit] Synopsis
Sarah runs into Fred Blorth, a man with whom she went to high school. At brunch, Laura and Jay announce to Sarah, Brian, and Steve that Jay will be receiving a Humanitarian of the Year award for the work that he does with blind children. Sarah attempts to upstage Jay by announcing to everyone that she allowed Fred Blorth to live in her home. Sarah recalls a story about how Fred was so embarrassed by his lunchlady mother's constant "queefing" and that the embarrassment caused him to be committed to a mental asylum. Sarah is invited to appear on the television show Valley View Live to discuss her humanitarian work that she has done with Fred, but she begins being visited by a ghost that tries to warn her. Sarah ignores the ghost's warnings, and she appears on the show and begins mocking Fred's mother and her habits (Vaginal flatulence). Fred watches this at home and becomes very angry. When Sarah gets home, Fred begins trying to kill her with a knife.
Meanwhile, after Brian and Steve are beaten and mugged two times, Steve begins to criticize Brian for not defending him by putting to use the skills that he learned in his karate classes. Later on, a man knocks on their door and punches Steve in the face; but once again, Brian fails to defend him.
Brian comes over to Sarah's apartment to borrow something, but he has to use his karate skills to defend Sarah from Fred. In the end, the ghost(who turns out to be Fred's mother) warns Sarah that queefing will render Fred powerless, and this allows Brian to defeat Fred, and Fred is taken away by the police.

- In this episode, Jay's last name is revealed to be McPherson.
- It is revealed in this episode that Sarah, Laura, and Brian all attended Valley Village High School, along with homeless man Fred Blorth. However, the school does not actually exist; and residents of Valley Village, California, attend North Hollywood High School.
- Brian is seen wearing a Titannica T-shirt. Titannica is the name of a fictitious band in Mr. Show. Brian Posehn played the lead singer of the band. Many of the actors were on this 90's HBO sketch comedy show, including Sarah and Jay
- While Brian is using his karate on Fred Blorth the theme to The Last Dragon is playing.
- In the fight scene, a Homestar Runner poster can be seen in the background.
- Sal Iocano, the actor who plays the neighbor who punches Steve in the face, is the cousin of Jimmy Kimmel, Sarah Silverman's real-life boyfriend. Iocano is a regular on The Man Show and Jimmy Kimmel Live, two shows hosted by Kimmel, as well as taking over for Kimmel after he left his post as the host of Win Ben Stein's Money.
- Guest stars Jill Talley and Doug Benson are the fourth and fifth Mr. Show actors to appear on the show. The first three were regulars Sarah Silverman, Brian Posehn, and Jay Johnston.
