[edit] Part 1

The episode begins with Carla arguing with Turk and JD about a painting that is above the crib and its violence involving Optimus Prime fighting Megatron. Carla then reveals to the audience that Turk still cuddles with his mother when he gets scared causing Turk to make an offensive term about Carla's dead mother, causing Carla to force Turk to go to her grave and ask for forgiveness. After Turk gets back he states his concern about having a baby. All this talk about mothers inspires JD to start filming a video to show his mother his work place, with the occasional insult to the fact he is wearing a camera on top of his head. He then meets up with Dr. Cox who tells JD to come with him to a patient with locked-in syndrome who can only talk with a computer. Dr. Cox begins to insult JD along with the patient and eventually the patient’s computer breaks. Reputably insulting JD, much to his chagrin. JD then goes outside to take a nap and is kidnapped by the janitor.

[edit] Part 2

This act starts with Turk and Carla talking about where JD is, and wait for him to come in. Instead of JD the janitor comes in talking to himself about what to do now since he has incapacitated JD, the user of most of his time. He is then questioned about where JD is leading to a shot showing JD being stuck in an outside boiler. The janitor then walks off after talking to Dr. Kelso. Kelso still standing there insults Dr. Cox who then tries to intimidate Turk but to no avail. And Turk in returns the insult, but is countered by Dr. Cox who mentions that Turk is very white for a black person, and does not consider Turk a black person. The janitor then comes back into the scene and starts to talk to the locked-in syndrome patient about his fondness for inventing and some of his other ideas. The scene then cuts away from him to Elliot talking to Dr. Kelso about her students making mistakes angering her. Carla comes in complementing Elliott's attitude and how much she hates Dr. Cox and horrible he was to her. It then cuts to the basket ball court where Turk tries to prove to Dr. Cox that he is black by playing basketball. Dr. Cox and Lonnie (who, Unbeknownst to Turk, was all-conference at Villanova) then start a game with Turk and The Todd. It cuts back to the janitor who is helping Carla with a patient bringing up his spirits. The Dr. who caused the issue is concerned as it was her mistake that caused the problem with the patient and how Elliot will be mad at her, which she now is. It cuts back to the janitor who is now narrating a bunch of events including him in high spirits and Turk being beaten in basketball by Lonnie and Dr. Cox, who makes fun of him for being a white black guy and Elliot getting mad at her newbie's and Carla taking the blame. The scene returns to the janitor who overhears people talking about how he is not very important, breaking his spirit.

[edit] Part 3

The final act starts with another one of janitor’s inventions and how he believes he is important. It then cuts to Dr. Elliot yelling at her interns about who screwed up. This scene also shows how much Keith obeys every whim of Elliot's. Carla then threatens Keith that she will rain punishment on him if he tells Elliot who was the Dr. that screwed up. It then cuts to Carla and Turk test driving a minivan in front of the hospital causing Dr. Cox to insult Turk again for being white. The janitor begins narrating a series of events, including the janitor breaking the patient’s computer and Keith meeting Elliott's crazy sex prone mother (Markie Post) scaring Keith into telling Elliot who made the mistake, Lisa. Turk starts to talk to Carla about being black and how he feels he is black, including the way that he is treated different from the other white members of the hospital. But he is concerned about changes the baby will make to his image and make him feel older. It cuts to Kelso yelling at the janitor because of his time mistake, he was massaged by a man instead of a woman, calling him useless. The janitor then turns to the patient and talks to him about why he chose to be a janitor because he feels that he is part of a team that helps people. It then cuts to Elliot talking to Dr. Cox about needing to be easier on her interns and how she didn’t do it all on her own and how Carla helped her while she was an intern. It then cuts again to Carla and Turk talking about how much a baby will change everything in their life. Turk is scared about it and Carla comforts him by explaining that it’s normal for people to have babies. It then does another cut to the janitor speaking to the patient about his life and that he is lonely. The janitor realizes it is 6 o' clock and lets JD out of the water storage. Montage roles with JD finally narrating, Elliot thanks Carla for her help and how she really helped her, Turk realizing he is having a baby and must deal with it. And the paralyzed patient thanking the janitor who, finally getting the respect he wants, walks off as the episode ends.

Spoilers end here.
