John Constantine arrives at a house in Los Angeles to preform an exorcism on a little girl. Notwithstanding preforming the exorcism, he notices something is greater than it seems; more specifically the type of demon that he was trying to get rid of. From here on out things get worse for John when he is told later from his smoking habits is fueling a cancer in his lungs. This fact forces him to relive the things he has done in the past. In the meanwhile, Angela Dodson, a detective who does not belive in the God or the Devil vows to find the true meaning of her sister's Isabel death. Angela does not seem to understand why her sister, being a deeply religous Catholic, want to kill herself.
The story further becomes intertwined when Angela discovers John's profession and tries to seek help from him. John's attitude at first was to tried to push her back but she persisted. In the end they discover that Isabel had similar powers from John that in being able to see demons and interact with them. Being institutionalized because of a lack of understanding, Isabel, unable to control herself killed herself. After John "renews" Angela's long buried powers she confirms that Isabel is in hell.
