Planescape popularized the word a little bit, giving it this definition: http://www.planewalker.com/encyclopedia/encyclopedia.php?intEntryID=119 "Factotums are the faction members that hold the lowest official rank within factions. Factions may give other names to their factotums as they see fit."

They would be beneath Factors: http://www.planewalker.com/encyclopedia/index.php?startsWith=F " Factor A Factor is the middle-man between Factol and Factotum in the factions of Sigil."
