How about not

formed the basis of Stephen Sondheim's musical of the same name, with book by Hugh Wheeler.

but

formed the basis of Hugh Wheeler's book for Stephen Sondheim's musical of the same name.

I think Steve's ego can stand taking seeming 2nd billing [wink] for the sake of logic & clarity, unless i'm missing something abt this situation.
