Spoiler warning: Plot and/or ending details follow.

The film opens with Alfie's seemingly perfect life, with everything he could hope for: a "Semi-Quasi sort of Girlfriend" - a single mother named Julie - and his many pieces of skirt on the side. Although his women rely on him and feel greatly for him, Alfie sees them as expendable and doesn't see the consequences of his actions. He casually goes through different women, including Lonette, the ex-girlfriend of Alfie's best friend Marlon. After resuming a relationship with Marlon, Lonette finds out that she is pregnant and decides to go for an abortion. Later, Marlon and Lonette move to the suburbs in order to start a new life together.

Alfie takes this and a mortal health scare to heart and deems "aim[ing] higher" to be his New Year's resolution. He strikes up a relationship with Liz, a mature woman who owns her own cosmetic empire. Although the relationship begins as a casual affair, Alfie slowly falls for her, assuming that she feels the same. Meanwhile, Alfie discovers through an unannounced visit on Marlon and Lonette that the couple's new baby is actually Alfie and Lonette's.

Alfie tries to cope by attempting to resume a relationship with Julie when he runs into her later on, only to discover that she's now happily involved with someone else. He then turns to Liz for reassurance and comfort, but finds her in bed with someone else instead. Shocked and dismayed by one blow after another, Alfie finally realises that his lifestyle would always have consequences.
