Chris Flinn (Harrington) is on his way to a job interview and is driving through the mountains of West Virginia. Due to a chemical spill in the road ahead, he decides to take a different route, leading him through an abandoned dirt road in the middle of the woods. He then crashes into a car stranded in the center of the road.
He soon meets a group of friends on a hiking trip, which includes Jessie (Dushku), whose car is broken down on the road. Two people stay in the car, whilst the remainder of the group goes for help, only to find an old cabin isolated in the West Virginia woods. They discover that the cabin has many grisly horrors inside, such as body parts in a fridge and entrails in the bathtub. The cabin is home to a group of cannibalistic mountain men, who are horribly disfigured due to generations of inbreeding.
The group realize that the ones who own the cabin will not be friendly. A confrontation erupts with the disfigured men and the group make a mad dash through the woods only hoping to make it out alive. They spend a couple days running from the mountain men, with two people from the group falling victim to the men. The remaining two people, Chris and Jessie, make it out of the woods alive and end up destroying the cannibals and their cabin, however at least one cannibal survives, as shown in the ending credits.

- In the scene when the four are running from the cabin after awaking the mountain men, Desmond Harrington broke his right ankle after landing on the opposite side of a log. This made it very difficult to shoot some of the scenes after his left leg is "shot" and he has to limp on his right leg.
- The United States promotion for this film was minimal due to the MPAA deeming the majority of TV Spots and Trailers "Too Intense" for the viewers. Even the commercials that were shown were heavily trimmed.
- During one of the last scenes in the movie, Eliza Dushku accidentally set actor Julian Richings (Three Finger) on fire.
- Several of the cast and crew were covered in rashes throughout the filming of the movie; this was due to the chairs being placed in what was first thought to be a group of weeds only later to be discovered as a batch of poison oak.
- Emmanuelle Chriqui dislocated her shoulder performing her fall through the trees. You can hear her shoulder pop on the production track in the theatrical sound mix.
- There are many similarities between Wrong Turn and Wes Craven's The Hills Have Eyes, and Texas Chain Saw Massacre.
- Zegers and Lindy Booth would later appear together and as boyfriend/girlfriend in the remake of Dawn of the Dead
- Chris Flinn's Desmond Harrington car is a 1967 Ford Mustang bearing Pennsylvania tags, suggesting that Pennsylvania is his home state.
- The double-headed eagle on the vest Eliza Dushku is wearing in the film is the national symbol of Albania. Dushku's father is of Albanian descent.
- The female lead character, Jesse Burlingame, was named after the heroine in Stephen King novel, Gerald's Game
