I encourage everyone involved in this project to also take a look at the disambiguation style proposal being discussed at Wikipedia:Disambiguation/Style. While that proposal is still under discussion, in its current form it would oppose formatting place names in the style you're proposing here on disambiguation pages. The draft style guide discourages linking any words other than the actual disambiguated pages.
Here's the rationale: someone (perhaps a Canadian) types in "London" into the search box, and it brings up (naturally) London, England. So they click on the See also London (disambiguation) link to look for the London, Ontario page. Having any other links on the disambiguation page other than those of alternative "Londons" would just be distracting. Dab pages aren't for exploration, they're for clarifying which page someone is looking for. Your input on this would be welcome at Wikipedia Talk:Disambiguation/Style.

Note:the following is slightly scarcastic and hostile. Please don't take personally.
So, instead of making simple, [[whatever the name of the article is]] links for articles on places, you want us to hide the name of the article and make it easier for people to mistakenly go to an article they didn't mean to go to, in order to... save one page load. Within the first 3 lines or so of every place article with a comma is a link to the place it's in(such as Ontario for your example). Sorry to come on so hostile, but I really don't understand. Is wanting to go to the article on Ontario when you see a link to London, Ontario a problem that you or someone you know has actually had? JesseW 04:15, 15 Jun 2005 (UTC)

problem
The problem with using this formatting is that it directly contradicts dab style (MoS:DP). Disambiguation pages aren't supposed to have links on them, except to the articles being disambiguated, nor are they supposed to have piping of links, because we're supposed to see the actual article name in question. It's not as important for dab pages, but for main articles I feel this kind of standardized location format is very important. In addition the nation should always be listed. You or I may know where Ontario is but I'm sure many people do not know it is in Canada, or might think it is itself a country, etc. I often see locations from countries with which I'm not familiar that should specify the nation. So the introductory example would instead be:
Here is an idea to make this easier. Each location article gets a template, e.g. {{London, Ontario}} that resolves to the fullly-qualified unambiguous wikitext, i.e. "London, Ontario, Canada". I proposed this on Village Pump earlier but didn't get much response. —Quarl (talk) 2006-01-06 23:52Z
