WV Route 49 is a state route that runs along the Tug Fork of the Big Sandy River, and the Norfolk Southern Railway line. The road begins at a turnoff from U.S. 52 at East Williamson. It then follows the Tug Fork to Delorme, WV, where it crosses over towards Phelps, KY.
