Sweet Tooth is a video game character from the Twisted Metal series of video games. A clown driving a combat ice cream truck, his face has been featured on the cover of every Twisted Metal game to date.

His age is unknown and his real name is supposedly Needles Kane. He is a killer clown, complete with a severely cracked clown mask, a red nose and polka dotted pants. During an execution by electric chair, another character in Twisted Metal: Black known as Preacher cursed him with the burden of the fires of Hell upon his head, which burn endlessly and cause him pain. He wants nothing more for it to be gone, but in order for that to happen Sweet Tooth would need to give up his hobby of randomly killing innocent people. He throws the cure away and kills the man who was offering it to him: Calypso. The above references to Twisted Metal: Black, which is revealed to be an illusion inside the mind of the real Sweet Tooth, and is not canon. Sweet Tooth drives the "Tasty Treats" Ice Cream Truck, in which he stores other "items" besides ice cream.

Needles Kane never really killed anyone, it was just a distorted illusion. In some games, although it is never actually said but is more than obvious, Needles Kane is the son of a fellow character Charlie Kane. It is also known that Needles is Marcus Kane's split personality. In Twisted Metal: Black, it is revealed that Needles Kane has a younger brother. In some games, Sweet Tooth is the main character, but in others he is merely a mascot, although easily one of the most popular characters in the game.

In the first Twisted Metal game, he is portrayed by an uncredited actor in live-action cutscenes. However, he lacks of his trademark flaming head.

Even though Needles drives Sweet Tooth as usual in "Twisted Metal Head-On", he also drives both Dark Tooth and Tower Tooth. In the game, it is said that Needles had convinced Marcus (His split personality) to join him in a dream. Dark Tooth was a result of this partnership, though in a previous title, Dark Tooth was driven by Needles Kane's father, Charlie Kane. Tower Tooth was just found and was used to convince Marcus.

Sweet Tooth is also an unlockable character in War of the Monsters (2003) for the Playstation 2, a game in which Incognito used the TMB engine to make a movie monster fighting game. One of the characters, Agamo, has Sweet Tooth as the fourth skin if you have a Twisted Metal: Black save on the memory card while playing the game. Sweet Tooth is represented by a tall mech with the signature flaming clown head, and often breaks out in his usual laughter throughout the game.
