Survivor Series 2004 was the eighteenth annual Survivor Series pay-per-view professional wrestling event for World Wrestling Entertainment. It took place on November 14, 2004 from the Gund Arena in Cleveland, Ohio. The official theme song was "Ugly" by The Exies.
