State Route 5 is an east-west highway in Ohio. Its western terminus is in Portage County at Interstate 76 near Ravenna, where it’s multiplexed with State Route 44, and its eastern terminus at the Pennsylvania state line, where Pennsylvania Route 58 (Jamestown Rd.) begins eastward. Portions of the route near Ravenna and near Newton Falls (where it intersects with Interstate 80 and the Ohio Turnpike) are 4-lane divide highways, and a 9-mile stretch from 3 miles west of Warren to Elm Road north of Warren is built to freeway standards. This alignment is not the original designation for State Route 5; its original alignment ran from the Indiana state line to the Pennsylvania state line and was almost completely replaced by U.S. Route designations in 1926 (see History).

  • 1932 – Route established along the former SR 36 alignment from Wooster to the Pennsylvania state line along the current SR 585, an unnumbered road through Akron, SR 59, and east from Ravenna (current Newton Falls Road) through Newton Falls (current Ravenna Road and Warren-Ravenna Road) and Warren (current Market Street and Elm Road) to its current alignment to the state line[2].
  • 1969 – SR 5 was routed around Ravenna and truncated at the I-76 (then I-80S) junction with SR 44; the segment from Wooster to SR 21 (then US 21) was renumbered as SR 585, and the segment from Akron to just east of Ravenna was designated SR 59[2].
  • unknown – Route from Ravenna to 0.7 mi. west of SR 82 in Braceville Township realigned to bypass Newton Falls and railroad crossings probably around the time that the interchange with I-80 and The Ohio Turnpike was constructed. Also realigned in Bazetta Township to avoid 2 crossings of the same railroad.
  • 1970 – Realigned along Warren Bypass from US 422 to Elm Road northeast of Warren and signed as SR 5B (Bypass)[2].
  • 1971 – Realigned along Warren Bypass from 3 miles west of Warren to US 422[2].
  • 1978 – Warren Bypass upgraded to freeway[2].
  • unknown – Warren Bypass designated as SR 5; SR 5B designation dropped.
  • 1979 – Warren Bypass extended eastward from Elm Road and designated SR 82; original bypass west of Elm Road cosigned SR 5/SR 82[3].

  • 1924 – Original route established[4]; originally ran from the Indiana state line 14 miles west of Van Wert to the Pennsylvania state line 6 miles east of East Liverpool along the following alignments[2]:
    • current US 30 from the Indiana state line to Mansfield.
    • US 42 from Mansfield to Ashland.
    • US 250 from Ashland to 5 miles west of Wooster.
    • US 30 from 5 miles west of Wooster to 2 miles east of Dalton.
    • SR 172 from 2 miles east of Dalton to eastern Canton.
    • US 30 from Canton to East Liverpool.
    • SR 39 from East Liverpool to the Pennsylvania state line.
  • 1926 – All above designations were changed to their current designations except for the segment from Delphos to Mansfield[2].
  • 1932 – Delphos-Mansfield alignment designated as US 30N[2].
