State Route 43 begins at its northern terminus in Cleveland, Ohio and crosses Ohio for approximately 130 miles to its southern terminus at an intersection with State Route 7, where it is known as the "Dean Martin Highway", along the Ohio River in Steubenville, Ohio.

It runs through the counties of Cuyahoga, Portage, Stark, Carroll and Jefferson. Its northern terminus is in Downtown Cleveland where it merges with State Route 14 as Broadway Avenue until it passes through the Slavic Village neighborhood. South of the Slavic Village the two routes split with State Route 43 becoming Miles Avenue and State Route 14 continuing along Broadway Avenue. State Route 43 crosses from Cleveland through Warrensville Heights and North Randall where it passes under the triangular interchange of Interstates 271 and 480 and U.S. Route 422. On the other side of the triangle, State Route 43 runs along Aurora Road from Bedford Heights into Solon where it intersects with State Route 91 (SOM Center Road) in downtown Solon before clipping the southwest corner of Geauga County and crossing into Portage County and Aurora, where it passes Geauga Lake Amusement Park. In downtown Aurora, State Route 43 merges with State Route 306, becoming Chillicothe Road as it turns due south and passes through Streetsboro and Kent. In Stark County, State Route 43 passes through Canton and in Jefferson County it reaches its southern terminus in Steubenville along the Ohio River and the Ohio-West Virginia border across from Weirton, West Virginia. The southern terminus occurs at the intersection of State Route 43 and State Route 7, named the "Dean Martin Highway" after the Steubenville native, famed as a movie star, crooner and member of the Rat Pack.
