Sam and Dean Winchester find Jo Harvelle and her mother Ellen staging a heated argument at the Roadhouse Saloon. Jo has been investigating the mysterious disappearance of six women in a Philadelphia apartment building. When the Winchester brothers go to investigate the apartment, they find two things that surprise them: ectoplasm and Jo, who is trying to rent an apartment. After covering for Jo with her mother, Dean tries to talk with Jo about the consequences of becoming a hunter. Dean then finds blond human hair in a ventilation grille after smelling an unknown substance.

The next morning, another girl has disappeared. This time, it's Teresa Ellis, and Jo becomes more personally involved with the hunt, which she claims to be doing for her father. Everyone begins researching, and Jo finds an old photo of the empty lot the apartments were built on. A prison can be seen next door, and the trio discovers that prisoners were hung in the empty lot. On the list of prisoners is H. H. Holmes, America's first serial murderer according to the Winchesters, who liked to capture young blond women using chloroform (the smell Dean had noticed earlier) and keep them for days in hidden chambers in the walls. But his corpse is buried under concrete, making it unreachable.

Hoping to find Teresa, Dean and Jo investigate inside the walls of the apartment building. When they reach a point too narrow for Dean to slip through, Jo goes on alone. Dean tries to take another route to reach her, communicating with her via mobile phone. Jo screams, and a frantic Dean sledgehammers through the apartment building wall only to find Jo's phone on the ground and no hidden rooms. That night, Ellen calls Dean, finding out about the hunt through Ash. Dean reluctantly tells her that Jo is missing, and an angry Ellen says she's taking the first flight out to find her daughter.

Jo wakes up in a small chamber to find herself trapped, but Teresa is alive in a container across the room. Jo tries to reassure her when Holmes' ghost reappears, ripping out a chunk of Jo's hair. When he returns, trying to touch Jo again, she stabs him in the hand with her father's pure silver knife to drive him away. Meanwhile, the Winchesters discover an old, unused sewage system underneath the city that they believe Holmes is using to hide his victims. They stumble on the room where Jo and Teresa are being held just as Holmes is about to knock out Jo with chloroform.

After driving Holmes off with rock salt shells from their shotguns, the brothers free the two women and get Teresa to safety. They use Jo as bait to draw Holmes back to the chamber, where they encase him in a circle of salt. Dean steals a cement truck, using the cement to fill in the old sewer to contain Holmes forever. They then drive back to the Roadhouse Saloon with a very angry Ellen in the front seat. Back at the bar, Ellen and Jo have another argument. Ellen tells Jo that she should not work with the boys because she holds John Winchester responsible for her husband's death. Jo reveals this to Dean in anger and severs ties with the brothers, telling them to leave immediately.
