New York State Route 386 is a north-south running highway located to the west of Rochester in Monroe County. Its southern terminus is at New York State Route 383 in Scottsville. The northern end of the highway is located at New York State Route 104 in Greece.

Route 386 begins at a three-way intersection with NY 383 on the west side of the village of Scottsville. From this junction, NY 383 heads south and east while NY 386 takes the west exit, passing under the Rochester and Southern Railroad and turning to the north as it encircles the village. After crossing North Road, which acts as an extension to NY 253, the route that ends at a nearby intersection with route 383, NY 386 becomes Scottsville-Chili Road, a name it retains for a considerable length of time. Shortly after this intersection, NY 386 crosses Interstate 90, the New York State Thruway, on an overpass.

Now in Chili, NY 386 intersects NY 252 just after crossing Black Creek. Further to the north, route 386 intersects and joins NY 33A on a short 1 mile duplex east to Chili Center, where the concurrency splits at an intersection that also includes the western terminus of NY 252A. NY 386 takes the north path out of the junction, becoming Chili Center-Coldwater Road.

Just north of Chili Center, NY 386 interchanges with Interstate 490 at exit 5. North of the exit, Chili Center-Coldwater Road comes to an end, forcing NY 386 to turn east onto West Side Drive for 1/2 mile before returning north onto Coldwater Road. On Coldwater, NY 386 crosses over the CSX mainline a shade under a mile before reaching NY 33 at a T-intersection in Gates. The two routes then share Buffalo Road for a brief distance before NY 386 resumes its northward trek on Elmgrove Road.

Proceeding north on Elmgrove Road, NY 386 passes the Rochester Tech Park and the Total Sports Experience prior to intersecting NY 531. This exit, the final on 531 eastbound before I-490, is also the starting point for the two service roads (each of which has a unique reference number) that parallel the expressway between Elmgrove and Manitou Roads. NY 386 then continues due north, intersecting NY 31 and crossing over the Erie Canal before reaching its northern terminus in Greece at NY 104 near Elm Ridge Plaza.

Originally, NY 386 consisted of only the Elmgrove Road section. However, when NY 251 was truncated from NY 33 to NY 383 in Scottsville, the 386 designation was extended along the former right of way of NY 251.
