New York State Route 145 is a state highway in New York. The highway runs from the Greene County town of Cairo to the Schoharie County town of Sharon. NY 145 is a two-lane highway its entire length, with a passing lane on hills leaving Middleburgh in both directions.

In Middleburgh, there is a short wrong-way duplex over the Schoharie Creek.

NY 145 may have started farther east on New York State Route 23, perhaps at the western terminus of NY 23's duplex with New York State Route 32. The first reference marker visible on NY 145 has digits 1 0 0 8, where 1 0 0 0 would normally be displayed at a terminus or county line.
