[edit] Episode overview

Jordan's siblings, Danni and Ben, return to help prepare a gathering for Dr. Cox's son's first birthday. Turk and Carla clash over removing Turk's mole, and changing Carla's last name after they marry, while one of Ted's band members quits. Dr. Cox angrily blames J.D. for a patient's death and dismisses him, but Ben calms Dr. Cox down and talks some sense into him, and he eventually forgives J.D.

It is revealed in the end that Cox was in denial. It was actually Ben who had died 20 minutes after Cox left him with JD; the gathering everyone prepares for turns out to be his funeral. Cox, devastated by the loss of his brother-in-law and best friend, tries to hold back tears during the ceremony, but fails.

[edit] Homage to The Sixth Sense

On the special features on the season three DVD, Bill Lawrence says that this episode is a homage to The Sixth Sense.

There are three clues to the twist ending, which are more understandable on repeat viewings.

First, when Cox asks Ben if he's still taking photographs, he responds "Until the day I die." After J.D. has broken the news to Cox that the patient has died (pronouns are used so that the audience can mistakenly think it was the recently introduced old man), Ben's appearances are without his camera.

Second, when Cox and Ben are walking through the hallway (after Ben's death), Cox tells Ben, "As a rule of thumb, I don't attend parties where the guest of honor has no idea what's going on." This is meant to lead people to believe he is referring to his baby son's birthday but actually he is referring to Ben's funeral.

Third, after J.D. has made the announcement, Dr. Cox is the only character to interact with Ben. Elliot looks at Cox curiously as he laughs at Ben acting as a puppeteer to her movements.
