The original Route 23 was one of the original 1922 Missouri highways. With the creation of the U.S. Highway System in 1926, it became part of U.S. Highway 67. Its northern terminus was at Route 9 at Fredericktown. Its southern terminus was at the Arkansas state line south of Neelyville.
