The Miss USA 2003 pageant was held in San Antonio, Texas in 24 March 2004. The pageant was won by Susie Castillo of Massachusetts, who was crowned by outgoing queen Shauntay Hinton of the District of Columbia.

The title of Miss USA 2003 was awarded to Susie Castillo of Massachusetts. The runners-up were:
- 1st runner-up - Michelle Arnette (Miss Alabama USA)
- 2nd runner-up - Nicole O'Brian (Miss Texas USA)
- 3rd runner-up - Elisa Schleef (Miss Michigan USA)
- 4th runner-up - Beth Hood (Miss Tennessee USA)
The ten semi-finalists were: Alicia Michioka (Miss Hawaii USA), Tashina Kastigar (Miss Indiana USA), Alina Ogle (Miss New Mexico USA), Star Williams (Miss Oklahoma USA) and Anna Hanks (Miss South Carolina USA).

- This year had one of the largest numbers of crossover delegates, and a record number of former Miss Teen USA contestants.
