Massimo went to college at Northwestern with Eric Forrester (John McCook), Stephanie Douglas (Susan Flannery) and Beth Logan (Nancy Burnett). He owns a shipping company that Eric suspects has connections to underground crime networks. Soon after his arrival in LA, Massimo revealed to Eric that he had been a confidante to Eric's wife Stephanie for years - since Eric and Stephanie's first divorce. Under the influence of his feelings for Stephanie, Massimo made arrangements for Brooke Logan (Katherine Kelly Lang) to leave and move to Paris. His interference caused animosity between himself and Stephanie and Eric's son, Ridge (Ronn Moss). When Ridge injured himself during an argument with Rick (Justin Torkildsen), hospital blood tests revealed that there was no possible way for Eric to be Ridge's biological father. Massimo reminded a shocked Stephanie of the time they were intimate in college. Stephanie concluded that Massimo was Ridge's biological father and begged Massimo to keep Ridge's paternity a secret. Massimo did so as long as he could.

Once the truth was revealed, Ridge soon opened his heart to his new father and left Forrester to join him at Marone Industries, much to Massimo's joy. Massimo was even more shocked when he learned he had a second son, Dominick "Nick" Payne (Jack Wagner), from a relationship with Jacqueline Payne (Lesley-Anne Down) years ago. Both of Massimo's sons fell in love with Brooke Logan. When Brooke chose Nick over Ridge (believing that Nick was the father of her baby), Ridge turned his back on Massimo, who had encouraged Brooke and Nick's union. Once the truth came out about Ridge being the child's father, Massimo learned that Jackie had known there was a chance that the baby could have been Ridge's. A furious Massimo blamed Jackie for Ridge turning his back on him. When he learned that Jackie had an affair, the strain was too much and Massimo had a devastating stroke. Confined to a wheelchair and unable to move or speak, a paralyzed Massimo was forced to watch while the manipulative Deacon Sharpe (Sean Kanan) tried to steal his wife right out from under him.

Massimo finally regained control of his life, and after hiring Heather Engle (Shari Headley) to make sure the once-alcoholic Deacon fell back off the wagon, he and Jackie reconciled. The reunion didn't last long, and they soon divorced. Massimo began getting closer to Stephanie again and wondered if they could have a future together, and was instrumental in Stephanie's finding the document that proved she was the rightful owner of Forrester Creations, which Eric had hidden from her during their entire marriage. He was later thrilled when he was given a chance to meet Dominick, the son Felicia Forrester (Lesli Kay) had given birth to nine months after leaving LA (and Nick) behind. However, a paternity test soon revealed that Dante Damiano (Antonio Sabato, Jr.), not Nick, was the baby's father.
