The second season starts off with Tommy at his new firehouse on Staten Island, although he is extremely bored because it gets few calls. Meanwhile, Sully, Tommy's replacement, is bonding with the rest of the old crew, which threatens Tommy's chances of being transferred back to the old firehouse, although Chief Reilly is pulling for him.

Tommy finds out that Janet took his kids to London, Ohio when Colleen calls him. Tommy pulls a few strings to join the firehouse's barbershop quartet to get to Ohio.

While responding to a warehouse fire call, the crew discovers that Sully is a cross-dresser into S&M, which eventually results in Tommy's transfer back to his old house. Tommy's father returns from his trip with a new wife he met in Ireland.

Damien, Sheila's son, says he wants to become a firefighter like his father, which prompts Sheila to get Tommy to try to talk him out of it. At the hockey game between the FDNY and NYPD, Tommy critically injures a player on the NYPD team to show he hasn't gone soft. After a weekend trip to Atlantic City, Uncle Teddy, Tommy's father, and Tommy find that his father's new wife is dead and that his father will soon be rich.

Tommy's estranged sister (played by Tatum O'Neal) returns for a brief time. The money Tommy's father thought he would inherit turns out to be far less than expected. Sheila is beaten up by her girlfriend. Jerry finds his wife on the floor of their bedroom bleeding from a cut to the throat. When the discovery of a lump on his testicle sends Mike for a checkup, his involuntary physical reaction to the male doctor causes him to question his sexuality. Connor Gavin, Tommy and Janet's only son, is killed by a drunk driver while on his bike.

Tommy seeks revenge on the man who killed his son, who has been involved in similar incidents, but has gotten off lightly because of his connections. Chief Jerry decides to put his wife in a nursing home in Connecticut. Laura abruptly transfers out of the firehouse. Lou discovers that Candy set up an elaborate plot to con him out of his money. Tommy, Uncle Teddy, and Tommy's father conspire to kill Connor's killer, and Teddy volunteers to do it. Tommy has a change of heart after Jesus visits him in a vision. The episode ends with Teddy shooting the man, with Tommy unable to stop him.
