[edit] Plot synopsis
The series picks up where it left off, with the bloodied bodies of Sam, Dean, and John Winchester in the wrecked Chevrolet Impala. The possessed trucker moves towards the Winchesters; Sam wakes up and threatens to shoot him with the gun, and the demon exits the trucker's body. The Winchesters are taken to a hospital. When Dean wakes up, he tries to find his family, but is seemingly ignored by hospital staff. He then finds his body still in a hospital bed; he is having an out-of-body experience. John and Sam try to plan their next course of action, while Dean moves around the hospital and encounters another separated spirit, Tessa.
Sam senses Dean's presence and obtains an Ouija board in an attempt to communicate with him; both are relieved when Dean is able to make contact. Meanwhile, John goes to the hospital basement and prepares to summon The Demon. When Sam finds a page on reapers in John's journal, Dean realizes that Tessa is a reaper trying to take his soul to the afterlife. John makes contact with the The Demon, and tries to strike a deal to trade The Colt for Dean's revival. The Demon, however, wants him to "sweeten the pot." It is not immediately clear what this means. After some conflict between Dean and Tessa, The Demon possesses the body of the reaper and forces Dean's soul back into his body, stating "Today's your lucky day, kid."
The next day, John tells Dean how proud he is of him and to take care of Sam, and whispers something to Dean. John leaves the room to relinquish the gun to an unknown person/spirit. Sam then finds his father collapsed on the floor; after calling for help, the doctors try to revive him, but fail.
