Horse Block Road is a main thoroughfare through central Suffolk County, New York. Its western terminus is at Middle Country Road (New York State Route 25) in Centereach. It then traverses to the south-southeast, and is cut off by Nicolls Road (Suffolk County Road 97), Suffolk County Community College, and other county facilities in Farmingville, and joins up with Portion Road (Suffolk County Road 16) from a frontage road west of Nicolls Road named Leeds Boulevard at the interchange with Nicolls Road. The section east of Nicolls Road resumes at a street called Horse Block Place and then joins Portion Road west of Waverly Avenue.

Horseblock Road then travels east-southeast through Farmingville, intersecting North Ocean Avenue (Suffolk County Road 83) and goes into Medford, intersecting New York State Route 112, the Long Island Expressway at Exit 65, and Suffolk County Road 101. It then enters Yaphank and Brookhaven, where it intersects Woodside Avenue (Suffolk County Road 99) and Yaphank Avenue (Suffolk County Road 21); where it shares an interchange with Sunrise Highway (New York State Route 27) before terminating at Montauk Highway (Suffolk County Road 80) in Brookhaven.
