In 1999, he was signed as an undrafted free agent by the Oakland Athletics. On December 18, 2003, Oakland traded Sierra and minor-league shortstop J.T. Stotts to the New York Yankees for relief pitcher Chris Hammond. Sierra made the Florida State League All-Star team the next season, working as a relief pitcher for the Tampa Yankees. He was traded again on July 28, 2005; the Yankees sent Sierra and Ramón Ramírez to the Colorado Rockies, receiving Shawn Chacon in exchange. Sierra's stay with the Rockies' organization was brief, as the Chicago White Sox claimed him on waivers on April 26, 2006.
