Drew vs. Billboard is the 47th episode of the sitcom The Drew Carey Show and is also the first episode of the third season.

At Winfred-Louder, Drew watches as Mr. Wick compliments another employee named Smith on his good health and assigns him to a business conference in Hawaii to represent the store. Drew believes that he was not asked to because he is out of shape and overweight. He attempts to show that he is fit by filling the water cooler when Mr. Wick asks for a volunteer, but Mimi ends up stalling, forcing Drew to have a difficult time holding the water jugs. At the Warsaw Tavern, Oswald is measuring Lewis's head, as he believes it is increasing due to something at DrugCo. Drew and Kate arrive and as Drew complains about being overweight, a nearby man named Morgan overhears him and offers him a pill called VitaBeer (the pill is ingested while drinking beer) to help him lose weight. Morgan offers to give Drew a free bottle of the pills if he is willing to take pictures of himself for the man's "before and after" campaign as long as he signs a contract. Drew is thankful to get the pills and can't wait to try them. When Lewis is not looking, Oswald switches his hat for a smaller one, making Lewis think that his head has significantly grown while at the bar.

Two weeks later, Drew is horrified to see a shirtless photo of himself on a billboard. The message on the sign reads "Honk if you want him to put his shirt back on!" and Drew is consistently being honked at. He becomes the laughingstock of the city, and cannot force Morgan to remove the billboard because he signed a contract for the free pills. At Winfred-Louder, Mimi continues the humiliation by bringing an air horn to the office and blasting it behind his back. When Lewis, Kate, and Oswald arrive to cheer up Drew, Lewis is wearing a mask made of ice, as he hopes it will shrink his head back to normal size. Due to the ice mask, during the conversation he continually "freezes" in his tracks. When a job applicant recognizes Drew from the billboard, he keeps his eyes fixated on Drew's belly. Drew gets angry and throws the applicant out of his cubicle, causing Mimi to call for Mr. Wick to punish Drew. Mr. Wick warns Drew that he is embarrassing the store and will be moved to the basement if he does not get the billboard taken down. To continue Drew's humiliation, Mimi reveals an ice sculpture of Drew showing his belly which she calls an "Ice-ca-pig" (in reference to Ice Capades).

In the middle of the night, Drew and the gang climb onto the billboard to tear it down. However, Morgan shows up to stop them, thanks to a warning from Mimi. The gang simply staple him to the billboard and pull his pants down to humiliate him for the way he humiliated Drew. Morgan warns that Drew has a suspicious mole after looking at the large picture, and Drew tears off the portion of the picture showing the mole and takes it with him. Back at Winfred-Louder the next morning, Mimi attempts to break into Drew's desk since he his gone for a doctor's appointment. When she opens the drawer, a pulley lifts her computer monitor up on a string, and when she closes the drawer, it causes the computer monitor to crash to the ground. Drew arrives to reveal to his fellow employees and the gang that his doctor said he was healthy. Mr. Wick tells Drew that Smith suddenly dropped dead of a heart attack while at the conference. He decides to send Drew to Hawaii to retrieve the body, but since he is only picking up the body he will not have any time for fun in Hawaii.

At the conclusion of the episode, Drew has fun by posing for photos with the corpse in various airports and shows the images to Kate, Lewis, and Oswald. Lewis then notices a check addressed to Drew and when they open it, they discover that Drew's "before" picture was used for a marketing campaign for a gay cruise.
